What goes in Box 15 of the CMS-1500?

A second dated event about the condition, identified by qualifier: an initial treatment date, a last seen date, or similar, used when a payer wants a specific companion date.

What does Box 15 mean for chiropractic billing?

Mostly payer specific. When a payer wants the initial treatment date for a chiropractic episode, this is where it rides, with the right qualifier. Absent an instruction, empty is correct.
