Before entering patient information
Contact us to arrange your practice’s subscription agreement and Business Associate Agreement (BAA), and to review how application data is handled. Use sample information for evaluation until the required agreements are in place.
An evaluation practice or viewing this page does not sign a BAA. This page describes the process; it is not a subscription contract or an acceptance record.
