Box 16: Dates patient unable to work

By Dr. Daniel Turner, DC · Reviewed September 2, 2026

What goes in Box 16 of the CMS-1500?

The from and to dates of work disability, when the provider is certifying time off work. Empty on the vast majority of chiropractic claims.

What does Box 16 mean for chiropractic billing?

Relevant when work status intersects the case, most often workers compensation or a PI case with wage loss claims. If you complete it, the chart must support it, because those dates get read by more than the payer.

How Cervica fills Box 16

Cervica generates the CMS-1500 and its electronic 837P equivalent from the chart and the payer's own rules: identifiers from the verified insurance record, modifiers and codes per payer, and pre submission scrubbing that flags exactly the mistakes listed above before a claim leaves the building.
