CMS-1500 reference

Box 7: Insured address

By Dr. Daniel Turner, DC · Reviewed September 2, 2026

What goes in Box 7 of the CMS-1500?

The policyholder’s address, or SAME when it matches the patient’s. Relevant mostly for dependents whose subscriber lives elsewhere, such as students or children of divorced parents.

What does Box 7 mean for chiropractic billing?

Low drama, worth accuracy: divorced parent plans and college students are exactly the situations where this differs from Box 5, and exactly the claims that already carry extra matching risk.

How Cervica fills Box 7

Cervica generates the CMS-1500 and its electronic 837P equivalent from the chart and the payer's own rules: identifiers from the verified insurance record, modifiers and codes per payer, and pre submission scrubbing that flags exactly the mistakes listed above before a claim leaves the building.
