KX modifier: Requirements specified in the medical policy have been met

Category: Modifier

By Dr. Daniel Turner, DC · Reviewed September 2, 2026

What is KX modifier used for?

The attestation modifier stating that the requirements of the applicable medical policy are met and the documentation proves it. In therapy billing it rides claims that exceed the annual therapy threshold, converting a hard stop into a documented exception.

How it works in a chiropractic practice

Reaches chiropractic billing through the payers that apply therapy thresholds to the rehab codes DCs bill. When a patient’s accumulated therapy spending crosses the annual threshold, continuing claims need KX to attest continued medical necessity, and the note has to be ready to back the attestation. Some commercial payers borrow the same mechanism for their own visit caps.

What the note must show

  • ✓Evidence of continued necessity: objective progress measures, updated goals, and why continued skilled care rather than a home program
  • ✓Awareness of where the patient stands against the threshold, which is a benefits tracking problem before it is a coding one
  • ✓KX only when the threshold is actually in play; unprompted KX on routine claims is noise

A worked example

A post surgical patient seeing both a physical therapist and a chiropractor accumulates therapy charges toward the same annual threshold. When the combined total crosses it, the chiropractor’s 97110 claims start denying until KX is added, attesting the documented necessity. The biller who did not know the PT spending existed learns the threshold is shared the expensive way.

Does Medicare pay chiropractors for KX modifier?

Defined in Medicare therapy policy around the annual threshold amounts, with targeted medical review above a higher second threshold. The chiropractic exposure is indirect but real wherever therapy codes are covered and counted.

Where practices get burned

Attesting without documentation that survives review; missing shared threshold accumulation across providers; reflexive KX that draws review attention.

How Cervica helps with KX modifier

Cervica's claim scrubber validates modifiers, units, and diagnosis pointers for KX modifier before submission, per payer rules handle code substitutions and modifier requirements automatically, and when a payer pushes back anyway the denial worklist reads the adjustment codes and routes the claim to the exact fix.
